Sermorelin Before and After: What Trials Measured Over Weeks and Months

The sermorelin studies published no photos. Here is what the studies did measure at 2 weeks, 6 weeks, 16 weeks and a year, and which changes never showed up.

If you are searching for sermorelin before and after, you probably want to know what changes, and when. The honest answer is that the studies tracked blood tests far more than bodies. None of them published photos.

What the studies did measure is still useful. It tells you what to expect first, and what may never show up. This page walks through it week by week. The bigger question of whether sermorelin works has its own page.

Before you start: what the studies used as a baseline

Sermorelin is the first 29 amino acids of growth-hormone-releasing hormone [5]. It signals the pituitary to release growth hormone. The liver then makes IGF-1, which carries many of growth hormone’s effects.

So the studies measured three things at the start. They took growth hormone in blood drawn through the night, IGF-1 in the morning, and in some cases body composition by scan. Those are the numbers that moved, or did not.

The first hours

The fastest change is a pulse of growth hormone. In a 16-week study of a modified form of the peptide, growth hormone rose within 10 minutes of the nightly injection [3]. The pulse lasted about 2 hours.

That study used [Nle27]GHRH(1-29), a close analog, not sermorelin itself. A pulse is only the first link in the chain. It says nothing yet about muscle, fat or sleep.

After 2 weeks

This is where the strongest sermorelin result sits. Ten healthy men with a mean age of 68 took 1 mg twice daily for 14 days [1]. Their 24-hour growth hormone and IGF-1 rose to the levels of a group of young men.

The lower dose, 0.5 mg twice daily, did not produce a significant change. Fasting glucose and blood pressure did not move on either dose. The study measured hormones only, not muscle or fat.

After 6 weeks

A second study changed the schedule. Eleven men aged 64 to 76 injected 2 mg once nightly for 6 weeks [2]. Nighttime growth hormone rose, but IGF-1 did not.

Weight, body mass index, and muscle and fat on a DEXA scan did not change. Two of six strength tests improved, the upright row and shoulder press, along with one endurance test. There was no placebo group to compare them against.

The authors concluded that single nightly doses are less effective than several a day. That matters if you are offered a single nightly shot.

After 16 weeks

The longest adult study used the modified analog again, in 10 women and 9 men aged 55 to 71 [3]. They injected placebo for 4 weeks, then the analog nightly for 16 weeks.

IGF-1 rose within 2 weeks and stayed up through week 12. By week 16 it was drifting back toward baseline in both men and women. Skin thickness increased in both.

Lean body mass rose in the men only. Body weight did not change, and sleep quality was unaffected in both groups. Men reported better well-being and libido; women did not.

After 6 months to a year

The longest sermorelin record is in children, the use it was once approved for. One trial enrolled 110 children with growth hormone deficiency (86 were analyzed). Nightly injections raised mean growth from 4.1 cm a year to 8.0 cm after 6 months [4]. At 12 months it was 7.2 cm a year.

At 6 months, 74% of the children were rated good responders. The trial had no control group. It tells you sermorelin can drive a real, measured change when growth hormone is truly low. It does not tell you what happens in a healthy adult.

What these studies could not show

Neither sermorelin study in adults ran a placebo group alongside the drug. The analog study gave its placebo first, to everyone. None of the adult studies ran active treatment for longer than about four months. The before-and-after photos online are not drawn from these studies.

This does not mean sermorelin does nothing. It means the changes most people hope to see were never tested in a way that could show them.

What to ask before you judge your own results

Ask for an IGF-1 test before you start and again after several weeks. That is the change the studies show can fail to appear. Ask what dose and schedule you are on, and how it compares with the ones above.

How long does sermorelin take to work?
In the studies, hormone changes came first. Growth hormone and IGF-1 rose within 2 weeks on 1 mg twice daily in older men. At 6 weeks of once-nightly 2 mg, growth hormone rose but IGF-1, muscle and fat did not change.
Are there real sermorelin before-and-after results?
