The NMN vs NAD question looks like a choice between two supplements, but the two molecules sit at different points on the same production line inside cells. NAD+ is the finished coenzyme that energy metabolism runs on, while NMN is one of the intermediates the body builds it from, and that difference shapes how each is sold, regulated and studied.
What each molecule is
NAD+ is a coenzyme for the redox reactions of energy metabolism, and a required partner for enzymes including sirtuins, CD38 and the poly(ADP-ribose) polymerases, which touch processes from DNA repair to immune function [1]. The same review notes that NAD+ levels in tissues fall gradually with age in rodents and in humans.
FDA’s own description of NMN is that it is an intermediate in NAD+ biosynthesis, produced from nicotinamide [9]. The team behind a 10-week NMN trial in women called its availability a rate-limiting factor in how mammals make NAD+ [2].
So the logic of taking NMN is indirect: supply more of the raw material and let cells finish the job. Taking NAD+ is the direct version, handing over the finished molecule and hoping it reaches the cells intact.

Why size comes up, and where it stops being evidence
NMN weighs 334.22 grams per mole, about half of NAD+ at 663.4 [7] [8]. That smaller size is often put forward as an advantage for crossing the gut wall and cell membranes.
That is where the chemistry runs out and hypothesis begins, because size alone does not show that swallowed NMN reaches tissues intact. The human trials below measured blood levels and clinical markers rather than tracking the molecule itself into cells.
NAD+ given straight into a vein does not stay intact for long either. In a pilot study of a six-hour NAD+ infusion, plasma NAD+ and its breakdown products, NMN among them, showed no change for the first two hours [6]. The authors concluded the infused NAD+ was rapidly and completely removed from plasma over that period. What else studies of the infused molecule recorded is covered in NAD IV therapy.
What human NMN trials measured
A 2020 safety study gave 10 healthy Japanese men single oral doses of 100, 250 and 500 mg NMN and followed them for five hours [3]. It found no significant symptoms or changes in heart rate, blood pressure or temperature, and two nicotinamide breakdown products in plasma rose with the dose.
A 60-day trial randomized 80 healthy middle-aged adults to placebo or 300, 600 or 900 mg NMN a day [4]. Blood NAD concentrations rose in every NMN group compared with placebo (all p ≤ 0.001), highest at 600 and 900 mg, and six-minute walking distance increased more than with placebo.
Insulin resistance, measured by HOMA-IR, did not differ from placebo in any NMN group in that trial. Three of its authors are employees of chemical or pharmaceutical companies, as the paper discloses.
A 10-week trial in postmenopausal women with prediabetes and overweight or obesity found that muscle insulin sensitivity, measured with a glucose clamp, increased on NMN but not on placebo [2]. That is one trial in one narrowly defined group.
What happens when the trials are pooled
A 2024 meta-analysis pooled eight randomized trials covering 342 mostly non-diabetic, middle-aged and older adults, at 250 to 2,000 mg a day for 14 days to 12 weeks [5]. It found no significant benefit on fasting glucose, insulin, HbA1c, HOMA-IR or blood lipids.
A 2026 meta-analysis of 15 trials reached a similar result on metabolism, with only a slight fall in diastolic blood pressure, and found no increase in adverse events or liver enzymes over up to 24 weeks [10]. Its authors call for larger, longer trials before drawing conclusions about efficacy.

What this means for someone choosing
The two options are not interchangeable products at different prices; they are different molecules that reach people through different legal routes, and the evidence behind each measures different things.
Oral NMN has randomized trials showing it raises blood NAD and is well tolerated over weeks, but pooled results do not show broad metabolic benefit. NAD+ itself has far fewer human studies, and they are set out route by route in NAD+ injections benefits.
