Revive Longevity

A Florida storefront that hands prescribing to Telegra MD's provider network and names the pharmacy that compounds your order.

Review by Molly Ward, goal-guide writer · Updated September 2026

The short version

You can see who does what before you pay. The terms say Revive Longevity does not practice medicine. Telegra MD's provider network prescribes, and Strive Pharmacy in Arizona compounds. The gaps are clinical: no product page prints a dose, and your clinician is not named in advance.

Revive Longevity LLC runs its own website from Hallandale Beach, Florida. It sells cash-pay compounded treatments for weight, longevity, hormones and sexual health. On the peptide side you will find tirzepatide, sermorelin, NAD+, glutathione and PT-141. NAD+ also comes as a nasal spray and a face cream. The rest of the shop is hormone care: TRT, HRT, HCG and enclomiphene, plus a lipotropic B12 shot. A Truck Driver Stack bundles tirzepatide, NAD+ and sermorelin for people who drive for a living.

What Revive Longevity prescribes, by goal

Weight

  • TirzepatideFDA-approved
    • Injection (compounded)From $299/mo

Compounded tirzepatide is a once-weekly injection, with no strength printed. A one-month supply is $299. The 4-week subscription is $279, but it bills 13 times a year, so it costs more over twelve months. The page sets a BMI floor of 30, or 27 with a weight-related condition. The FAQ says your price stays the same regardless of your dosage.

Sexual health

PT-141 is $199 every 4 weeks. The page says pharmacies make it as an injection or as a nasal spray. It never says which one you receive, because your provider picks the formulation. It is candid about risk. It warns of blood pressure rises and says any use in men is off-label.

Longevity

  • NAD+Legal to compound
    • Nasal spray (compounded)From $159 per 4 wks$172 a calendar month, our arithmetic
  • GlutathioneLegal to compound

Listed by the seller; no price we hold.

NAD+ Nasal Spray is $159 every 4 weeks, used typically once daily. No strength is printed. The main NAD+ product is $179 every 4 weeks, but its route depends on your provider's protocol. That could mean an injection under the skin, into muscle, or an IV infusion. Glutathione costs the same and also leaves the formulation to your provider. The NAD+ Face Cream may use a precursor such as nicotinamide instead of NAD+ itself.

    Recovery

    • SermorelinLegal to compound
      • Injection (compounded)From $179 per 4 wks$194 a calendar month, our arithmetic

Sermorelin is a daily injection under the skin, usually at bedtime, the page says. It is $179 every 4 weeks, or $199 for a one-month supply. No dose or vial size is printed. Your provider tailors the dose to your labs, including IGF-1, your age and your goals.

    Sleep

    • SermorelinLegal to compound
      • Injection (compounded)From $179 per 4 wks$194 a calendar month, our arithmetic

Each “From” figure is the seller’s own starting price, in the unit the seller states. Where a required fee or a package length changes what one month costs, the smaller line under it is our arithmetic, not a figure the seller prints. Compounded preparations are not FDA-approved.

    Who prescribes

    A licensed clinician

Revive Longevity is a front for the clinical side, and its terms say so plainly. They read: "Revive Longevity does not practice medicine, prescribe medications, or provide clinical care." Prescribing goes to Telegra MD, LLC. The terms describe it as running a national network of affiliated professional entities. Your clinician comes from that network and is not named before you start. The site features Diana Medina, PA-C, as its lead clinical reviewer and under "Meet your provider". It does not say whether she writes prescriptions herself. Strive Pharmacy in Gilbert, Arizona, is named as the compounding pharmacy. Revive Longevity itself takes your payment.

You start with a short online wellness quiz, which the how-it-works page puts at about five minutes. A licensed clinician then reviews it, and that page labels the step asynchronous. The service-areas page also mentions a virtual consultation. The consent form lists messaging, audio and video as possible formats, without saying which one you get. Your card is authorized at checkout and charged only once a provider approves your plan. After that, the product pages promise ongoing access to your care team for dosing and side-effect questions. No schedule for that follow-up is published.

    Are labs required?

    Care team decides

The FAQ calls lab analysis an important part of your plan and leaves the call to your care team. Draws happen at Quest centers, booked through the member portal. Blood panels are included only in the two higher membership tiers, two or four a year.

What it claims, and what it cites

The homepage leads with a weight figure: members lost an average of 29.3 lbs in their first six months. The footnote says the number is self-reported by 111 members using GLP-1 injections. It sits beside a citation to SURMOUNT-1, the trial of brand tirzepatide. That trial did not test this compounded product. The how-it-works page says the site is LegitScript-certified. The homepage badge links to LegitScript's own lookup for the domain. The product pages are careful about status. Each says the compounded version is not FDA-approved. The PT-141 page spells out its blood-pressure warnings in full. The homepage also promises you can receive your prescription the same day. The shipping policy puts most deliveries at 5 to 10 business days after approval. One legal page is unfinished. The terms still carry a bracketed note asking a healthcare attorney to add state-specific disclosures.

Sold by Revive Longevity, not listed here

NAD+ (the main product), Glutathione and PT-141
Each page leaves the route or formulation to your provider, so none can sit on a board that compares one form with another.
    NAD+ Face Cream
The page says it combines NAD+ or a precursor such as nicotinamide, so it may hold no NAD+ at all.
    Truck Driver Stack
A bundle of tirzepatide, NAD+ and sermorelin in one order, not a single product.
TRT, HRT, HCG, enclomiphene and Lipotropic + B12
Hormone and vitamin treatments outside the peptides this site covers.

    How it bills

Every product card shows three ways to pay. A 4 Week Subscription bills every 4 weeks, which is 13 charges a year. A 1 Month Supply is a one-time charge, and so is the 3 Month Supply. The subscription card strikes through the one-time price to show a saving. Over a year that saving can vanish: tirzepatide at $279 thirteen times costs more than twelve months at $299. The how-it-works page says the price covers the consultation, prescription, medication and shipping. Standard shipping is free. You can cancel future renewals anytime, by email or in the patient portal. Once the pharmacy starts compounding, there are no refunds. Canceling after provider review but before compounding can cost up to $50. A separate membership menu runs from $299 to $999 every 4 weeks. It bundles products, labs and coaching, and each tier starts with a booked call.
