As semaglutide and tirzepatide became common, people began describing breasts that looked smaller, emptier or lower after the weight came off. A 2026 plastic surgery review notes that “Ozempic breasts” is “not a medical or clinical term” but is used for changes in breast size or shape after large weight loss on these drugs [1].
The question has two parts. Does the drug itself act on the breast, and what happens to a breast when a large share of body fat is lost by any route? The same question about the face has one imaging study behind it, covered in Ozempic face.
What the labels say
The Wegovy and Zepbound labels mention the breast only in their breastfeeding and lactation sections [2] [3]. Neither lists a change in breast size, shape or feel as an adverse reaction.
What both labels do say is that the drug “lowers body weight with greater fat mass loss than lean mass loss.” That matters for the breast, because of what it is made of.
Why breasts get smaller with weight loss
The 2026 review states that “breast tissue is primarily composed of fat,” and that the loss of fat after massive weight loss, by any method, “leads to dramatic changes in the breasts” [1].
The clearest numbers come from a 2017 study of 80 women who had mammograms before and after weight loss surgery [4]. Their mean body mass index fell from 46.0 to 33.7.
Software read breast volume from the mammograms. Mean volume fell from 1,470 to 890 cubic centimeters, a 39.4% drop. The dense gland tissue fell far less, from 70.5 to 59.6 cubic centimeters, or 15.5%.
Volumetric density at the start was 5.15%, so gland was about one twentieth of the breast, and most of what was lost was fat. That is a surgical population losing more weight than most drug-trial participants, so the size of the change should not be carried over directly.
| Study | Who | What was measured | Finding |
|---|---|---|---|
| Vohra 2017 | 80 women after weight loss surgery | Breast and gland volume on mammograms | Volume down 39.4%; gland down 15.5% |
| Williams 2017 | 110 women after weight loss surgery | Breast area and density | Breast area and percent density fell |
| Hosseini 2019 | 42 women after weight loss surgery | Reported density category | Over a third changed; 93.3% of changes were increases |
| Sun 2023 | 7 studies, 535 people | Density, pooled | No change by radiologist category; higher by volumetric software |
What happens to breast density
Density on a mammogram is the share of the breast that is gland and connective tissue rather than fat. When fat leaves faster than gland, that share can rise even as the breast shrinks.
In the 2017 volume study, mean volumetric density rose from 5.15% to 7.87% after surgery [4]. A San Francisco cohort of 42 women found over a third changed density category, and 93.3% of those changes were increases [5].
Not every study agrees. A cohort of 110 women found percent density and breast area both fell after surgery [6]. A 2023 meta-analysis of seven studies with 535 people explains part of the split [7].
Scored by radiologists' categories, density showed no significant change (odds ratio 1.27, 95% CI 0.74 to 2.20). Scored by volumetric software, it rose. The authors conclude the result depends on the method used.
A Dallas cohort found density rose in 14% of women after surgery. Its authors note that higher density can mask a tumor, and that some patients may benefit from extra screening based on risk scores that include density [8].
Shape, sagging and surgery
From the bariatric literature, the 2026 review lists what massive weight loss can do to the breast: “severe ptosis, asymmetry, and reduced lateral curvature, as well as loss of upper pole volume” [1]. Ptosis means sagging.
For GLP-1 drugs specifically, the authors write that such changes are “not reported in the GLP-1 RA literature.” They describe patients from their own practices with wrinkled chest and breast skin and nipple ptosis, and present two case studies.
The review grades its own level of evidence as 5, and its GLP-1 material rests on two surgeons' experience. A 2024 claims-database study found a correlation of r = 0.28 between Wegovy use and subsequent breast procedures, among 59 Wegovy users [9]. It shows a correlation in billing data, not a measured change in the breast.
What this means
The measured part is simple: breasts are largely fat, and fat is what these drugs remove. How much a given breast changes on the drugs has not been measured.
The size of that loss on semaglutide is in how much weight you can lose on semaglutide. The lean-tissue side of the same loss is in GLP-1 muscle loss.
